COMES NOW, SARA YVONNE WARD, on behalf of DESARAE RUBY-ANN
POLLARD, by this petition and being duly sworn, respectfully represents unto this
Honorable Court as follows:
1. That the Petitioner, SARA YVONNE WARD, is the Mother of the minor.
2. DESARAE RUBY-ANN POLLARD was born on AUGUST 26,1998 in
HARRISBURG, Pennsylvania.
3. The minor child presently resides at 4 DULLES DRIVE WEST APT G-5,
CAMPHILL, Pennsylvania, 17011. Minor child has lived in Cumberland County,
Pennsylvania for 2 year(s) immediately prior to filing this petition.
4. The minor child's present name is DESARAE RUBY-ANN POLLARD. This is
the name on her social security card and all other records.
5. That DESARAE RUBY-ANN POLLARD wishes to change her name to
DESARAE RUBY-ANN WARD.
6. Petitioner desires this change because: HER FATHER KNOW LONGERS
WANTS TO BE A PART OF HER LIFE. I THINK IT IS BEST SHE TAKES ON
MY NAME.
7. The proposed change of name will not be detrimental to the interests of any other
person and is not against the public interest.
8. The petition is not being filed to defraud creditors.
9. That the Child has not been known by another name.
10. The mother of the child is SARA YVONNE WARD who resides at 4 DULLES
DRIVE WEST APT G-5, CAMPHILL, Pennsylvania, 17011.
11. The father of the child is CURTIS POLLARD who resides at UNKNOWN.
12. It is in the best interests of the child to change her name.
WHEREFORE, your Petitioner prays that this Honorable Court pass an Order changing
the child's name from DESARAE RUBY-ANN POLLARD to DESARAE RUBY-ANN
COMMONWEALTH OF PENNSYLVANIA

AND NOW, this 2nd day of December, 2002, upon consideration of the attached
Petition To Change Name of a Minor, a hearing is scheduled for Monday, February 24,
2003, at 3:30 p.m., in Courtroom No. 1, Cumberland County Courthouse, Carlisle,
Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
Statutory Notes] [Main Volume[), and 54 Pa. C.S. §§701 et seq.
PETITIONER shall make a good faith effort to set-ye the father of the minor child
with notice of the hearing and file a proof of service or attempted service in that regard.

AND NOW, this 24th day of February, 2003,
upon consideration of the Petition To Change Name of a
Minor filed at No. 02-5662 CIVIL TERM, and following a
hearing at which the natural father of the minor did not
appear to contest the petition, and at which the
Petitioner, Sara Yvonne Ward, the mother of the minor,
presented evidence to support the petition, the petition is
granted, and the name of Desarae Ruby-Ann Pollard (date of
birth, August 26, 1998) is hereby changed to Desarae
Ruby-Ann Ward.
