New York 14203. I am duly authorized to submit this claim for the Bank which is based upon my personal knowledge and/or upon
records kept in the ordinary course of the Bank's business.
2. The decedent is indebted to the Bank in the amount set forth in paragraph 3 below. The anlount claimed is correct as of the
date of death after all appropriate payments or other credits. There are no set-oITs or counterclaims. This claim is free from any
charge forbidden by applicable law and excludes all unmatured interest.
