PETITION FOR CHANGE OF NAME
Petitioners, Richard Steven Woods and Jami Lynn Woods, by their attorney, Timothy J.
O'Connell, Esquire, requests that this court order a change of petitioners' names, pursuant to 54
Pa. c.S.A. 9 701, et seq., and in support thereof makes the following averments:
1. Petitioner, Richard Steven Woods, is an adult individual who resides at 2204 Yale
Avenue, Camp Hill, Cumberland County, Pennsylvania.
2. Petitioner is and has been a resident of Cumberland County for at least 30 years.
3. Petitioner's current surname is that of his stepfather.
4. Surname ofpetitioner's biological father is Llewellyn.
5. Petitioner has no outstanding judgments or legal actions pending against him. A
certificate of judgment search is attached hereto. Petitioner has no prior criminal records.
6. Petitioner wishes to change his name to Richard Steven Llewellyn
7. Petitioner, Jami L. Woods, is the lawful spouse of Richard Steven Woods and resides
at 2204 Yale Avenue, Camp Hill, Cumberland County, Pennsylvania 17011.
8. Petitioner is and has been a resident of Cumberland County for at least 5 years.
9. Petitioner Jami Lynn Woods was lawfully married to petitioner, Richard Steven
Woods on June 13,2004. The parties have two children, Daniel Steven Woods, born October
15,2004, and Liam Duncan Woods, born March 9,2006.
10. Petitioner has no outstanding judgments or legal actions pending against her. A
certificate of judgment search is attached hereto. Petitioner has no prior criminal record.
11. Petitioner wishes to change her name to Jami Lynn Llewellyn.
WHEREFORE, petitioners respectfully requests this court enter an order changing their
names from Richard Steven Woods to Richard Steven Llewellyn and Jami Lynn Woods to Jami
Lynn Llewellyn.

the attached petition of Richard Steven Woods and Jami Lynn Woods, for change of name, and
upon presentation of proof of publication of notice as required by law, together with proof that
there are no judgments or decrees of record or any other matter oflike character against
petitioner, and it appearing that there is no lawful objection to the request ofthe petitioner, it is
hereby ORDERED that the name of petitioner Richard Steven Woods is changed to Richard
Steven Llewellyn and the name of petitioner Jami Lynn Woods is changed to Jami Lynn
Llewellyn. The names ofthe parties' two children, Daniel Steven Woods and Liam Duncan
Woods shall also be changed to Daniel Steven Llewellyn and Liam Duncan Llewellyn.
