PETITION FOR CHANGE OF NAME
NOW COMES Your Petitioner, SHANE MICHAEL RESANOVICH, by his
attorney, Charles E. Petrie, and respectfully represents as follows:
1. That Your Petitioner is SHANE MICHAEL RESANOVICH, an adult
individual currently residing at 226 Spring Lane, Enola, County of
Cumberland, Pennsylvania.
2. That Your Petitioner desires to change her name to SHANE MICHAEL
REZZO. Your Petitioner desires to change his name from a long and
cumbersome name to a shorter name to make it easier for others to spell and
pronounce.
3. That during the past five years, Your Petitioner has resided at the
following addresses: from January, 2002, until August, 2005, at 112 barkers
Road, Jonestown, County of Lebanon, Pennsylvania; from August, 2005, until
the present at 226 Spring Lane, Enola, County of Cumberland, Pennsylvania.
WHEREFORE, Your Petitioner respectfully requests that Your Honorable
Court pursuant to the Act of April 18, 1923, P.L. 75, Section 2, as amended, 54
P.S. Section. 2, to:
(a) fix a hearing date for this Petition; and
(b) direct that notice be given of the filing of this Petition and of the
hearing.
