MUNICIPAL CLAIM
AND NOW, comes the Claimant, South Middleton Township Municipal Authority, by
the Chairman of the Board of the South Middleton Township Municipal Authority, who files the
following claim.
1. The claim is filed against Jody L. Warrick, owner, (hereinafter "Owner") and all
that certain tact of land including improvements thereon, owned by her and described as follows:
1404 Trindle Road
Carlisle, PA 17013
Tax Property Map No: 22-0119
Parcel No: 004
Assessment District: 40
(hereinafter "Premises").
2. The claim is in the amount of $1,377.87 due and owing for sewer and/or water
services rendered for the time period of June 15, 1999 through November 6, 2002, for which
stun, with interest and costs, a lien is claimed.against the Premises in accordance with the
Municipalities Authorities Act of 1945, May 2, 1945, P.L. 382, 53 P.S. § 301, et seq., as
amended and the Municipal Claims and Tax Liens Act, May 16, 1923, P.L. 207, 53 P.S. § 7101,
et seq., as amended.

3. The fees claimed are based upon appropriate resolutions lawfully adopted by
South Middleton Township Municipal Authority (hereinafter "Authority").
4. Attached hereto is a statement of the kind and character of services so rendered
and the prices charged therefor, said statement being a true and correct copy of the Customer
History File for Owner's account with Claimant for the time period set forth above.
5. The Authority has incurred and will continue to incur attorney fees in the
collection of this delinquent account and claims those fees as a lien against the Premises.
