Notice is hereby given that the Plaintiff in the above matter, LAURIE A. RHODES, having
been granted a Final Decree in Divorce from the bonds of matrimony on the 17th day of November,
1994, hereby elects to retake and hereafter use her maiden name of LAURIE A. LECLERC, and
gives this written notice avowing her intention in accordance with the provisions of 54 Pa.C.S.
Section 704.
