NOW COMES Jacqueline Brandt and petitions this Honorable Court as follows:
1. Petitioner Is Jacqueline Brandt who resides at 929 Herman Drive, Mechanlcsburg,
Cumberland County, Pennsylvania.
2. Petitioner Is the mother of Sarah L. Brandt, a minor.

3. The named minor Is under 14 years of age having been born on May 25, 1986 In
Cumberland County, Pennsylvania.
4. The minor resides with Petitioner.
5. The minor's father, Carroll A. Brandt, died on January 4, 1994.
6. The minor Is entitled to certain property, specifically the proceeds of her father's life
Insurance polley with Northwestern National Ufe Insurance Company which, on the day of death,
carried a death benefit of One Hundred Fifty-three Thousand Six Hundred and 00/100
($153,600.00) Dollars, payable to the beneficiaries In equal shares, the beneficiaries being Petitioner
and the minor.
7. The date or death benefit, therefore, payable to the minor was Seventy-Six Thousand
Eight Hundred and 00/100 ($76,800.00) Dollars. Since the date of death, however, Northwestern
National ute Insurance Company has paid to Petitioner one-half of the benefit to Petitioner and

holds the other half In an Interest bearing account until a guardian of the estate of the minor can
be appointed by this Court,
8. The above named minor has now no general or testamentary guardian appointed by
will, deed or otherwise to the knowledge or belief of Petitioner. In addition to the proceeds of the
Insurance policy described above, the minor Is entitled to the following assets:
A. Assets deposited pursuant to the Uniform Gifts to Minor's Act, 20 Pa
C.S.A. Section 5301, et seq., Invested In mutual funds, and two bonds, all having a
present value of approximately Six Thousand and 00/100 ($6,000.00) Dollars.
B. Savings at the Defense Activities Federal Credit Union In an account with
Petitioner as custodian, the assets therein presently totalling Eleven Thousand One
Hundred One and 57/100 ($11,101.57) Dollars. The DAFCU Savings Account Is the
depository for the minor's payments from the Social Security Administration on
account of the death of her father from which account Petitioner makes occasional
withdrawals to provldEl for the minor's needs.
9. Petitioner requests the Court appoint Farmers Trust Company, of Carlisle, Cumberland
County, Pennsylvania, a corporation authorized to act as a fiduciary In the Commonwealth, to act
as the guardian of the estate of the minor.
10. The proposed guardian Is a fit and proper person to be appointed guardian.
11. The proposed guardian has no Interest adverse to that of the minor.
WHEREFORE, Petitioner requests that a decree Issue, appointing Farmers Trust Company
of Carlisle, Pennsylvania, the guardian of the estate of the above named minor and authorizing the
