He is married to the mother, but they are separated. 4. The relationship of plaintiff to the child is that of mother. 5. The relationship of defendant to the children is that of father. Plaintiff has not participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another court. 7. CUMBERLAND COUNTY, PENNSYLVANIA NO.<('l -\~5 CIVIL TERM JOHN CRAWFORD, DEFENDANT CUSTODY CU.S'l'OPV ORDER AND NOW, this .1(1"- day Of/(!.,,,__;., 1996, upon consideration of the parties' Consent Agreement, the following Order is entered with regard to custody of the parties' child, Corinne Marie Crawford, date of birth, August 5, 1993: 1. The plaintiff, Cynthia Crawford, hereinafter referred to as the mother, and the defendant, John Crawford, hereinafter referred to as the father, shall have shared legal custody. 2. The mother shall have primary physical custody. 3. The father, shall have partial custody according to the following schedule: a. Six weeks each summer, to be divided into periods of partial custody no greater than three consecutive weeks and no less than one week. After the child begins school, this period of custody shall occur during the child's vacation from school. The father shall give the mother at least two weeks notice as to when he will exercise this period of custody. b. Other times as agreed upon by the parties. 4. The mother and father shall alternate the following holidays at times to be mutually agreed upon by the parties: New i ~ Years Eve and New Years Day, Easter Weekend, Memorial Day Weekend, Fourth of July, Labor Day Weekend, the four day Thanksgiving holiday, Christmas Eve and Christmas Day. 5. The non-custodial parent shall have the right to see the child on her birthday at a time to be mutually agreed upon by the parties. 6. The mother and father shall share responsibility for transportation in a manner to be mutually agreed upon by the parties. 7. The mother and father, by mutual agreement, may vary from this schedule at any time. a. The parties agree to provide each other with reasonable telephone access to the child at all times. If the custodial parent takes the child on vacation or is staying with the child at a place other than the custodial parent's residence, the custodial parent shall provide the non-custodial parent with the address and telephone number of the place where the child is staying. 9. Each parent shall notify the other parent immediately of medical emergencies which arise while the child is in that parent's care. 10. The parties acknowledge that the raising of a child when the parties live separately carries with it special problems and obligations, and each party hereby agrees to cooperate with the other to the maximum extent possible to assure that the best lnterests and welfare of the child are served. from school.
