upon hearing on the within Petition and upon motion of Michael R,
Rundle, Esquire, attorney for the Petitioner, and upon proof of
publication of notice as required by law, together with proof
that there are no judgments or decrees of record or of any other
matter of like effect against Justine Autumn Miller, and it
appearing that there is no legal objection to the granting of the
prayer of the Petitioner,
IT IS ORDERED AND DECREED that the name of Justine Autumn
Miller, a minor, is hereby changed to Justine Autumn Zeiders,

6. The father of Justine Autumn Miller is Andrew S. Book,
whose last known address is 24 East Countryside Drive, Boiling
Springs, Cumberland County, Pennsylvania.
7. Andrew S. Book has had no contact with Justine Autumn
Miller in more than four (4) years, although he does pay support
to the petitioner for his daughter through the Cumberland County
Domestic Relations Office.
8. The Petitioner and her husband have a child born of
their marriage, which child has the surname Zeiders.
9. It would be in the best interest of Justine Autumn
Miller to have her name changed to Justine Autumn Zeiders for the
following reasons:
A. Her mother and half-sister have the surname
Zeiders;
B. Justine has resided in the Zeiders' household for
more than four years;
C. A change of name to Zeiders fosters the concept of
a family unit.
D. Justine has never had the surname of her biological
father.
10. There are no judgments or decrees of like character of
record against Justine Autumn Miller.
WHEREFORE, the Petitioner requests this Honorable Court,
pursuant to the Act of December 16, 1982, P.L. 1309, No. 295,
Section 2, 54 Pa. C.S. ~702 to do the following:
A. Fix a hearing on this Petition;

Robin G. Zeiders hereby verifies that the facts set forth in

the foregoing Petition are true and correct to the best of her
knowledge, information and belief, and understands that false
statements herein are made subject to the penalties of 18 Pa.
C.S. 54904 relating to unsworn falsifications.

RE: Change of Name of Justine Autumn Miller
Dear Mr. Book:
please be advised that Robin G. Zeiders has filed a petition with
the Court of Common Pleas of Cumberland County, Pennsylvania
requesting that the name of your daughter, Justine Autumn Miller,
be changed to Justine Autumn Zeiders. A hearing on that Petition
has been scheduled for Wednesday, April 2, 1997 at 9:00 a.m.
before the Honorable Harold E. Sheely in Courtroom No.1,
Cumberland County Courthouse, Carlisle, Pennsylvania. At the
hearing of said Petition, any person having lawful objection to
the change of name may appear and be heard.
