Inc. is the plaintiff in the above-captioned action which is currently pending in the Circuit Court of the Thirteenth Judicial Circuit of the State of Florida for Hillsborough County at Case No, 93-4396, Division No. I, 2, petitioner desires to take the deposition of Tom Ginnick, a material witness in the above-captioned action, who is currently located in Cumberland County, Pennsylvania, 3, Accordingly, Petitioner respectfully requests that this Court issue a subpoena duces tecum for the deposition of the following person: Tom Ginnick, United Telephone Company of New Jersey, Inc" 1201 Walnut Bottom Road, Carlisle, Pennsylvania 17013-0905, 4. The proposed deposition is to take place on or around February 5, 1997 at the offices of Central Pennsylvania Reporting Services, 54 West Pomfret Street, Carlisle, Pennsylvania 17013 or at a time, date and place otherwise agreed to by the parties and the witness, The witness will be required to bring with him the following documents: 1.
