following is a statement:
1. The Plaintiff, Ronald P. Pizur, Jr., Is an adult individual residing at 839 Pennsylvania Avenue.
Lemoyne, Cumberland County, Pennsylvania 17043.
2. The Defendant, Karen M. Pizur, Is an adult individual residing at 839 pennsylvania Avenue, Lemoyne,
Cumberland County, Pennsylvania 17043.
3. The Plaintiff and Defendant have been bona fide residents In the Commonwealth f'Or at least six months
previous to the filing 'Of this CDmplalnt.
4. The Plaintiff and Defendant were married on September 21,1991, in Scranton, Lackawanna County,
Pennsylvania.

6. The Plaintiff has been advised 'Of the availability of marriage counseling and that the PlaIntlffmay have
