4, Prior to this correspondence. Petitioner had no knowledge that her insurance
coverage was being canceled, having ncvcr rcccived a noticc of cancellation from the
insurance company,
5, Petitioner never knowingly rctilscd to pay thc prcmium so as to result in the
cancellation of her insurancc on thc abovc vehiclc
6, Immediately upon hcr rcccipt of thc Dcpartmcnt's corrcspondcncc, Pctitioner
sought and obtained insurance covcrage on the vchicle Irom ,\IIState Insurance Company
under policy numbcr 008323330, This coveragc was reinstated eficctive December 12,
1996,
7, Following the reinstatcmelll of hcr insurance coverage, Petitioner notified the
Department, through her insurance company, that coverage had been reinstated, on
December 23, 1996, A copy of AllState Insurance Company's correspondence is attached
hereto and incorporated by relercncc as Exhibit "13",
8, By correspondence dated January I, 1997, Petitioner was notilied by the
Department that her vehicle i'egistration privilege was being suspended lor a period of
three months for failure to provide proof of Iinancial responsibility, 1\ copy of this notice
is attached hereto and incorporated by reference as Exhibit "C",
9, This January I, 1997 correspondencc Irom the Department indicated that
Petitioner had been previously requested by the Department to provide proof of Iinancial
responsibility, and that either no response was received, or the information provided
indicated a lapse of coverage which was not permitted under Section 1786 of the Vehicle
Code,
