At the special instance and request of Defendant,
Plaintiff provided advertising in its publication known as
the "Donnelley Directory". After applying all proper
credits, there remains a principal balance due of S934.80,
as set forth in the attached sC:atement of aceount. As
provided in the attached advertisement contract, Plaintiff
is also entitled to interest on the above amount from
12/14/95 to 08/14/96 at the agreed rate of 18% per annum
totalling S112.01 and its attorneys fees in the amount of
S282.64, for a total of SI,329.45, which, despite demand
by Plaintiff, remains due. Defendant was required to
forward all payments to Plaintiff in Philadelphia
establishing a proper basis for venue in this eounty.
