From November 1995 through Deeember 1996, the child resided with the
plaintiff/mother, and her maternal grandparents, William and Sarah Jaeoby, at 15 E,
Countryside Drive, Boiling Springs, Cumberland County, Pennsylvania;
From December 1996 to present, the child resides with the plaintiff/mother, and
plainti1rs roommate, David Peneek, at 204 North Baltimore Avenue, Mt. Holly
Springs, Cumberland County, Pennsylvania.
4. The mother of the child is plaintiff, CINDY MCHENRY. She is married
to the defendant.
5. The father of the child is the defendant, DONALD B, MCHENRY. He is
married to the plaintiff.
6, The plaintiff is the ehild's biological mother, Plaintiff resides at 204
North Baltimore Avenue, Mt, Holly Springs, Cumberland County, Pennsylvania, with
her roommate, David Pencek,
7. The defendant is the child's biological father, Defendant resides at 40
East North Street, Carlisle, Cumberland County, Pennsylvania.
8, Plaintiff has not participated as a party or witness, or in another capacity,
in other litigation coneerning the custody of the child in this or another court.
9, Plaintiff has no information ofa eustody proceeding coneerning the child
pending in a court of this Commonwealth,
10. Plaintiff does not know of a person not a party to the proceedings who
has physical custody of the child or elaims at this time to have custody or visitation
rights with respect to the ehild.

Stipulation by the parties' in this maLter, the COUl't being satisfied that the best
interests of the minor ehild, EMMA JANE MCHENRY, will be served hereby, IT IS
HEREBY ORDERED AND DECREED as follows:
1. Legal eustody of the minor child, EMMA JANE MCHENRY, born May
28,1992, shall be shared by the parties, plaintiff CINDY MCHENRY, residing at 204
North Baltimore Avenue, Mt, Holly Springs, Cumberland County, Pennsylvania,
17065, and defendant DONALD B. MCHENRY, residing at 40 East North Street,
Carlisle, Cumberland County, Pennsylvania, 17013,
2, EMMA JANE MCHENRY shall reside with Plaintiff, who shall have
primary physical custody of the child.
3, DONALD B. MCHENRY shall have partial physieal custody of his
daughter for the purpose of visitation, on a regular and frequent basis, at times to be
determined upon the mutual agreement of the parties.
