Solely as a result of the negligence and recklessness of the Defendant,l'
David L. Krantz, the Plaintiff has suffered and In the future will
continue to suffer physical pain and mental anguish and loss of life's
pleasures, same to continue for an indefinite time in the future.

The matter is alleged to exceed the applicable arbitration limits and
a jury trial is hereby demanded.
WHEREFORE. Plaintiff, Dianna L. Cooke respectfully requests this I
Honorable Court to enter judgment agains the Defendant, David L. I
Krantz, in excess of Thirty Thousand ($30,000.00) Dollars plus interest

Solely as the result of the negligence and recklessness of the
Defendant, David L. Krantz, the Plaintiff Dana L. Cooke suffered
injuries to her neck, back, legs, headaches, pain into the shoulders,
headaches, and aggravation of pre-existing spina bifada condition. and I
a severe shock to her nerves and nervous sys ~m, which injuries are
serious and permanent.

As a further result of this accident, Plaintiff has been obliged to
receive and undergo medical attention and care and to expend monies
and incur expenses and may be obliged to continue to expend such sums
or incur such expenditures for an indefinite time in the future.
