presentation and consideration of the Petition of Marjorie I.
Straley, Executrix of the Estate of Paul J. Straley, for an Order
designating a Successor Custodian in the matter of Jeremy J.
Straley, a minor, it is ordered:
That Marjorie I. Straley is designated and appointed as
Successor Custodian of the accounts of Jeremy J. Straley, at the
IBM Employees Federal Credit Union, 610B Carlisle Pike, Suite 120,
Mechanicsburg, PA 17055, under member #17532 and social security
#164-56-7273.

5. The funds were deposited with the IBM Employees Federal
Credit Union under member #17532 and social security number of
Jeremy J. Straley, 164-56-7273.
6. Paul J. Straley became custodian of the account after the
death of the minor's father in 19BB pursuant to Order of the
Circuit Court for Montgomery County, Maryland. See copy attached.
Paul J. Straley remained as custodian of the account for Jeremy J.
Straley until July 26, 1994, when he died without having designated
a successor custodian.
7. No successor custodian of such accounts has been
designated under the provisions of the Pennsylvania Uniform
Transfers to Minors Act, 20 Pa.C.S. S5301, and Jeremy J. Straley,
who has not attained the age of 21 years, has no guardian in
Pennsylvania to become successor custodian.
B. Jeremy J. Straley consents to the appointment of Marjorie
I. Straley as successor custodian. A form of consent executed by
Jeremy J. Straley is attached hereto.
9. Your petitioner, Marjorie I. Straley, is the paternal
grandmother of Jeremy J. Straley. Both parents of Jeremy J.
Straley predeceased Paul J. Straley.
10. No previous application has been made for the relief
sought in this action.
WHEREFORE, your petitioner requests that this Honorable Court
issue an order under the provisions of 20 Pa.C.S. S5301 et seq.,
designating MARJORIE I. STRALEY successor custodian of such
accounts for Jeremy J. Straley and for such further relief as the
Court finds to be in the best interest of Jeremy J. Straley, a
minor.
