The Defendant(s), Justin Miller, is/are (an) adult individual(s) residing at the address indicated in the caption hereof. 2. On or about 12/05/95, Defendant(s) executed and delivered to Plaintiff a/an Note and Security Agreement under account number 10200128827600001. The sum of $7,335.50 was financed at a 15.5% rate. 3. Under the terms of the Note and Security Agreement, a copy of which is attached hereto and made a part hereof, the Defendant(s) was/were required to make prompt and regular payments. 4.
