Petition for Apllroval of Minor's Settlement
I. Your petitioner. Stanley Morrison, is the father and guardian of Ann M.
Morrison, a minor, and he resides at 1100 Longs Gap Road, Carlisle, PA 17013,
2. Your IXltitioner, Marilyr. T. Morrison is the mother and guardian of Ann M.
Morrison, a minor, and they reside together at 1075 Rebeca St., Carlisle, PA 17013
2, On October 29. 1994, the minor plaintiff, Ann M, Morrison, was injured as the
result of a motor vehicle accident, which occurred at the intersection of Spring Road and G
St, in Carlisle, Cumberland County. Pa,
3, As a result of the accident, which occurred on the aforesaid date, the minor child
was treated at the Carlisle Hospital Emergency Room by Dr, Anthony], Guarracino,
4, Dr, Guarracino released the aforesaid minor child from his care on October 29,
1996, A copy of his reports and bills are auached as Exhibit A.
5, The parties are desirous of concluding this maUer and have agreed upon a
settlement as follows:

$7.500,00
6, Plaintiffs are hereby authorized to sign a general release in favor of defendants,
Matthew and Diane Richards, and United Services Automobile Association,
7, All medical bills have been paid on behalf of the minor piaintiff by their fIrst
party benefits carrier, Attorneys fees will be paid independent of this settlement by United
Services Automobile Association.

WHEREFORE, your petitioners pray that the settlement of $7,500,00 be
approved, and the said sum of $7,500,00 be placed in a separate federally insured account,
marked, "Not to be withdrawn until age 18, or on further Order of Court,

We, the parents and natural guardians of Ann M, Morrison, a minor, born October 30, 1981, do hereby
state that the said Ann M, Morrison is completely recovered from any and all injuries sustained
as a result of an automobile accident involving Lori Morrison in a 1987 Pontiac Grand Am owned by StaiUey
Morrison, and Diane Richards at or near Spring Road and G St., Carlisle, Pa., on October 29, 1994,
We further state that said Ann M, Morrison does now eat, sleep and act normally and behaves normally
in aU respects in a normal manner, We are sure that a full and complete recovery has occurred.
WE HAVE READ THE ABOVE STATEMENT AND IT IS TRUE AND CORRECT,
