PETITION FOR NAME CHANGE
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
1. Petitioner, Alexander James Leech, is an adult
individual residing at 324 W. Main street, Mechanicsburg,
Cumberland County, Pennsylvania.
2. Petitioner has resided at the aforementioned address
since February of 1993 and has been a resident of Cumberland
County since at least 1990.
3. Petitioner is married to Anne Marie Manning, M.D.,
having been married on October 19, 1965.
4. Five (5) children have been born of this marriage.
5. Petitioner's wife is a physician and practices
locally in Cumberland County.
6. After their marriage, Petitioner's wife elected not
to change her name but retained the use of her maiden name in
her professional practice and in all of her legal affairs.
7. Petitioner and his wife have given all of their
children the last name of "Manning" on their birth
certificates.
6. Petitioner is desirous of taking the last name of

his wife.
9. A change of Petitioner's last name to "Manning" will
provide consistency in family matters and prevent confusion
when dealing with the public.
10. There are no judgments or decrees of like character
against the Petitioner.
11. It would be in the best interest of Petitioner
Alexander James Leech to change his name to Alexander James
Manning.
WHEREFORE, Petitioner prays the Court to enter a decree
changing the name of ALEXANDER JAMES LEECH TO ALEXANDER JAMES
MANNING.

IN REI PETITION FOR NAME CHANGE
ORDER OF COURT
AND NOW, to wit, this 27th day of May, 1997, upon
consideration of the petition filed and after a hearing, and
upon motion of Stephen K. Portko, Esquire, attorney for the
Petitioner, and upon presentation of proof of publication of
Notice as required by law, together with proof that there are no
judgments or decrees of record or any other matter of like
effect against Petitioner, with the exception of a mortgage in
favor of Maryland National Mortgage Corporation recorded in
Mortgage Book 1187, Page 745, in the Office of the Recorder of
Deeds in and for Cumberland County, Pennsylvania, and it
appearing that the petition is filed for a lawful purpose and
that there is no objection to the granting of the prayer of the
petition, it is ordered and decreed that the name of Alexander
James Leech be and is hereby changed to Alexander James Manning.
Petitioner is directed to promptly contact the
Maryland National Mortgage Corporation or any assignee thereof
to inform it of this name change and to comply with any requests
that it may make with respect to the mortgage as a result of the
name change.
