No. 2951(b) for judgment by confession and avers the following: 1. The Plaintiff is Dauphin Deposit Bank & Trust Company, which is headquartered at 213 Market Street, Harrisburg, Dauphi County, Pennsylvania 17101. 2. Defendants are Roger A. Hoover, Charles L. Hoover and Shirley L. Hoover, individually, jointly and d/b/a Charley-Roge Bowling Center with a business address of 34 Corporation Street, Newville, Cumberland County, Pennsylvania 17241. 3. Attached hereto and incorporated herein is a true and correct copy of the Commercial Loan Note ("Attached Instrument") duly executed by Defendants. 4. The Attached Instrument has not been assigned. 5. Judgment has not been entered on the Attached Instrument in any jurisdiction. 6. Default was made by the Defendants, individually and jointly in their failure to comply with the terms of the Attached Instrument and their failure to pay the full outstanding balance due and payable on the Attached Instrument upon maturity on May 1, 1996 and thereafter. 7.
