COMPLAINT
1. Plaintiff is a corporation with offices in 8604 Allisonviile Road, Indianapolis,
Indiana 46250.
2. Defendant is an adult individual residing at 16 Creekside Lane, Carlisle,
Cumberland County, Pennsylvania 17013.
3. Defendant is indebted to Piaintiff for sums due and owing as a result of a balance
due on an installment loan.
4. Defendant applied for and was granted an installment loan by Plaintiff, a true and
correct copy of the Loan Application is attached hereto, marked as Exhibit "1" and made a part
hereof.
5. Plaintiff is the holder of a Promissory Note evidencing said loan which Defendant
executed and delivered to Plaintiff on or about December 7, 1 994 in the principal sum of
$5,356.53, a true and correct copy of said Note is attached hereto marked as Exhibit "2" and
made a part hereof.
6. Defendant has defaulted under the terms of the Promissory Note having not made
payment to Plaintiff as promised, thereby rendering the entire balance due and payable.

WHEREFORE, Plaintiff demands Judgment in its favor and against Defendant, James E.
Myers, individualiy, in the amount of $4,961.51 with continuing interest thereon from March 11,
1997 at the rate of 15.99% per annum plus costs.
THIS 15 AN ATTEMPT TO COLLECT A DEBT AND ANY INFORMATION OBTAINED SHALL
BE USED FOR THAT PURPOSE.
