Perich, has, for at least four (4) months, been inoapable of managing and caring for herself. 7. That Harriet A. Perich exhibits symptoms of mental incapacity, including but not limited to, absent-mindedness, eccentrioity and senility. 8. That Harriet A. Perich's mental incapacity prevents her from managing and caring for the affairs of her person and estate. 9. That less restrictive alternatives are not available in this matter because of her condition. 10. That Harriet A. Perioh's condition has deteriorated in that: A. Her appearance and hygiene have gotten worse; B. She has feces under her finger nails; c. She has overwhelming body odor; D. She is dirty and disheveled; E. She appears disoriented and confused;and F. She 5ppoars agitated. 11- That a Geriatrio Assessment hae been oonduoted of Harriet A. Perioh whioh has resulted in a diagnosis of senile-onset dementia with possible psyohotio features, i.e., suspioiousness and paranoia. 12. That a reoommendation made by Roger J. Cadieux, M.D., Assooiate Professor of psyohiatry, and Director of the Geriatrio Assessment Program at the Milton S. Hershey Medioal Center, refleots that Harriet A. Perioh has a degree of oognitive impairment that would be oonsidered moderate to severe which requires 24 hour supervision to insure her safety, and that appointment of a GuardJ,an of her person and estate would be appropriate. 13. That the Petitioner has been advised that Harriet A. Perich hae the following physical conditions: a. Canoer. of the breast and thyroid gland b. Chronic Hypocalcemia; c. cyponatremia; and d. possible diabetic predisposition 14. That less restrictive alternatives are not available because her children are unwilling to care for her. 15. That Barry LNU (Last Name Unknown), considerably younger than the alleged incapacitated person, has acoess to her house and his relationship to Harriet A. Perich is not clearly known. 16. That the approximate gross value of the estate of Harriet A. Perich i8 not completely known at this time but it is believed that she is the owner of her residence. 17. . That the petitioner is unaware of the amount of money received by Harriet A. Perich each month from social Seourity and other souroes. lB. That the Petitioner believes and, therefore, avers that a Plenary Guardian of the Estate of Harriet A. Perich should be appointed. 19. That Harriet A.
