TO THE HONORABLE JUDGES OF SAID COURTI
PETITION FOR CHANGE OF NAME
AND NOW, BLANCHE G. COOK, a/k/a B.G. Cook, by and through her
attorney, Clayton W. Davidson, Esquire, respectfully requests that
this Honorable Court order a change of name pursuant to 54 Pa.
Cons. Stat. S701 et seq., and in support thereof represents the
following I
1) Petitioner, BLANCHE G. COOK, a/k/a B.G. Cook is an adult
individual residing at 6 Abbey Lane, Camp Hill, Cumberland County,
Pennsylvania, 17011.
2) Petitioner contends she has resided at this address for
the past ten (10) years prior to filing this petition.
3) Petitioner desires to have her named changed to BRIE G.
COOK for sentimental reasons. Petitioner was raised by her great-
grandmother, whose given name was Brie, and Petitioner would like
to honor her great-grandmother by having her name changed.
4) Petitioner is fifty-one (51) years old, having been born
in Elizabeth, New Jersey, on April 20, 1946 as BLANCHE MORIN.
(Petitioner's birth certificate is attached hereto as Exhibit A.)
5) In September 1978, Petitioner was married to Anthony P.
Cook in Dauphin County, Pennsylvania.
