The petition of Linda A. Carnes and Judy E. Hoch, children of the alleged incompetent,
respectfully represents:
I. Holy Spirit Hospital has custody of the alleged incompetent at 503 North 21st Street,
Camp Hill, PA 17011.
2. The alleged incompetent was admitted to the aforesaid institution on October I, 1994.
3. The alleged incompetent is domiciled at 112 East Simpson Street, Mechanicsburg, PA
17055.
4. The alleged incompetent is 73 years of age, having been born on December 28,
1920.
5. The alleged incompetent's military status is: Never served.
6. The alleged incompetent's marital status is: Widowed.
7. Those persons, if any, who are the alleged incompetent's next of kin and their
relationship to same, of whom your Petitioner has knowledge are as follows:
Linda A. Carnes and Judy E. Hoch.
8. No other Court within this Commonwealth of which Petitioner has knowledge has
appointed a guardian for the alleged incompetent.
9. The alleged incompetent is incapable of handling her financial and personal affairs.
13. TIle petitioners, having no interest adverse to the alleged incompetent, has agreed to
act as Guardian if this Honorable Court shall so appoint.

(a) $12,000.00 held by Harris Savings Bank in the form of a Certificate of Deposit.
(b) $ 3,500.00 in personal property located at the Incompetents apartment.
II. The alleged incompetent has debts outstanding In the amount of $ 9,800.00,
as of October 5, 1994, consisting of:
(a) $ 9,800.00 Loan due to Harris Savings Association.
12. The alleged incompctentls incapable of handling her personal and financial affairs for
the reasons set forth an Affidavit and Competency Report, prepared by Dr. Kevin L.
Stockton, to be marked as Exhibit A and submitted to this court prior to hearing.
13. The petitioners, having no interest adverse to the alleged incompetent, have agreed
to act as Co-Guardians if this Honorable Court shall so appoint. The consent of the proposed Co-
Guardians to so act is attached hereto and made a part hereof.
14. If appointed as Co-Guardians, Petitioners are required to act in compliance with
regulations promulgated under Court Order in 5 Pennsylvania Bulletin 931 et seq. (April 19,
1975).
