Pennsylvania, 4. The authorities under and by virtue of which the assessment as levied, arc the Municipality Authorities Act of May 2,1945, P.L, 382, as amended and supplemented; Resolution of Hampden Township Sewer Authority No. 96-02, adopted August 14, 1996 as amended, The claims for collection of fees are based upon the Act of the Assembly approved May 16, 1923, P,L. 207, as amended and supplemented, 5. All acts, conditions, events and things required to be done under the Ordinance of the Township of Hampden, Resolutions of the Hampden Township Sewer Authority, and Acts of the Assembly of the Commonwealth of Pennsylvania, have been and were performed in due and legal form, to entitle the Hampden Township Sewer Authority to file a lien for the payment of this claim. 6. The date of completion of the Project as certified by the Consulting Engineer, duly appointed by the Hampden Township Sewcr Authority, was August 14, 1996. 7. The kind and character of work done for which this claim is filed is construction of a sanitary sewer system in Hampden Township, Cumberland County, Pennsylvania, for the recovery of part of the cost for which this claim is made, Hampden Township Sewer Authority levied, assessed and charged part of the cost and expenses of construction upon and against properties benefited, improved or accommodated thereby, in accordance with the aforesaid Ordinance of the Township of Hampden and Resolution of Hampden Township Sewer Authority, by assessment for front foot rule or benefit rule, as appropriate, 8. Notice of assessment and the intention to file this claim if not paid, or if installment payments were elected, was duly given on or about October 15, 1996, 9. The payment of Two Hundred Twenty-eight and 57/100 Dollars ($228,57) was due on February 3, 1997, when the premises was connected to the Collection System, 10. The claim is filed for a total sum One Thousand Six Hundred Dollars ($1,600,00), being the unpaid balance of the fee due for the Contribution Fee, with interest thereon at the rate of eight percent (8%) per annum from February 3, 1997 plus docket costs, HAMPDEN TOWNSHIP SEWER AUTHORITY Dated:A-':1,~~~ By: Henry F.
