Ewideh, residing at, 11 Partridge Circle, Carlisle, PA, Cumberland County. PA, 17013. She is married. The father of the children is Arafat Maswadeh, residing at 335 N. West Street, Carlisle, Cumberland County, PeMsylvania, I 7013. He is married. 5. The relationship of Plaintiff to the children is that off ather. The Plaintiff currently resides with the following person(s): Name None 6. The relationship of the defendant to the children is that of mother. The Defendant currently resides with the children and the following person(s): Name Relationship Michelle Logue Mother Linda Logue Sister 7. Plaintiff has not participated as a party or witness. or in another capacity, in other litigation concerning the custody of the child in this or another court. Plaintiff has no infonnation of a custody proceeding concerning the child pending in a court of this Commonwealth. Plaintiff does not know of a person not a party to the proceedings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 8. OODER OF CXXIRT AND!Oi, this consideration of the attached and directed as follows: ~ C:iO: Cnc:liation , 2000, upon Report, it is ordered 1. tt1e Father, Arafat Maswadeh and the Mother, Valerie R. Ewideh, shall have shared legal custody of Isaac Maswadeh, born June 1, 1997, and Alexus Nor Maswadeh, born July 17, 1998. Each parent shall have an equal right, to be exercised jointly with the other parent, to make all major non-emergency decisions affecting the Children's general well-being including, but not limited to, all decisions regarding their health, education and religion. 2. tt1e Mother shall have primary physical custody of the Children. 3. The Father shall have partial physical custody of the Children on all days when the Mother is working fran before the Mother leaves for work as arranged by agreement of the parties until 7:30 p.m., unless agreed otherwise by the parties. The parties' agreement to this partial custody schedule for the Father is based upon the Mother's current work schedule under which she works at least 40 hours per week, usually fran 3:00 or 4:00 p.m. until 11:00 p.m. The Mother shall provide the Father with her work schedule promptly upon receipt each week. 4. The parties shall make custody arrangements for the Children on holidays by agreement. tt1e Father shall be entitled to have custody for a 3 hour period on each Child's birthday. 5. Each party shall be entitled to have uninterrupted custody of the Children for up to 1 week during each surrmer upon providing at least 2 weeks advance notice to the other party. 6. In the event either party intends to remove the Children from his or her residence for an overnight period or longer, that party shall provide the other party with an address and telephone number where the Children will be staying. 7. Unless otherwise agreed between the parties, the Father shall be responsible to provide transportation for all exchanges of custody.
