5. The relationship of the Defendant to the child is that of Father. The
Defendant currently resides with the following persons: N/A
6. Plaintiff has not participated as a party or witness. or in another capacity,
in other litigation concerning the custody of the child In this or another court.
Plaintiff has no Information of a custody proceeding concerning the child pending
in a court of this Commonwealth.
Plaintiff does not know of a person not a party to the proceedings who has
physical custody of the child or claims to have custody or visitation rights with respect to
the child.
7. The best interest and permanent welfare of the child will be served by
granting the Plaintiff primary physical custody.

Report, it is
1. The Mother, Ladene Myers, and the Father, Mark C. Cocns,
shall have shared legal custody of John C. Cocns, born August 11, 1994.
2. The Mother shall have primary physical custody of the Child.
3. The Father shall have partial physical custody of the Child
every weekday (Monday through Friday) from 5:00 p.m. until 9:00 p.m. each
day. In addition, the Father shall have custody of the Child on
alternating weekends, beginning August 16, 1997 from Saturday morning
through Sunday at times to be arranged by agreement of the parties.
4. The parties shall alternate or share custody of the Child on
holidays as follows:
A. Christmas: In every year, the Father shall have custody of
the Child from December 23 at a time to be arranged by
agreement of the partieD through Christmas Eve at 9:00 p.m.
In every year, the ~lother shall have custody of the Child
from Christmas Eve at 9:00 p.m. until December 26 at 12:00
noon.
B. Alternating Holidays: The parties shall alternate having
custody of the Child on the following holidays: Easter,
Memorial Day, July 4th, Labor Day and Thi1flksgiving. The
times for exchanges of custody during the foregoing holidays
shall be arranged by agreement of the parties.
5. Each party may have an uninterrupted period of custody with
the Child for two (2) weeks (consecutive or non-consecutive) each year upon
providing thirty (30) days advance notice to the other party.
6. The Father may have additional periods of custody with the
Child as agreed by the parties.
7. The party receiving custody of the Child shall provide
transportation for the exchange of custody, unless arranged otherwise by
agreement of the parties.
8. This Order is entered pursuant to an agreement of the parties
at a Custody Conciliation Conference. The parties may modify the
