Before me, the undersigned Notary Public, this day,
personally appeared R. Barry Weldon, who acknowledged himself to
be the Manager of Plaintiff, and that as such Manager, being
authorized to do so, and being duly sworn according to law,
deposes and states as follows:
1. Defendant has defaulted under the terms of the Order of
Court dated December 20, 1997, in that Defendant has failed to
make the payment due April 17, 1997, or thereafter. Copies of
the payments made by Defendant are attached hereto,
2. Pursuant to the Order of Court dated December 20, 1997,
the outstanding balance due and owing by Defendant is $1,017.40
