Esquire. and MA'rI'HEW O. STANLEY. by aud through his allomey. James J. KaYl'r. Esquire, and
MARK AND JOAN SLACK. representing themselves, pm Sf who do slipulate amI agree upon the
following:
I. RACHEL L. STANLEY and MATI'HEW D, STANLEY are the nalural parents of one minor
child. AMANDA LEIGH STANLEY. born rehruary 1.1995.
2, Primary physical custody of the minor child. AMANDA LEIGH STANLEY. shall be placed
in the maternal grandparenls, MARK AND JOAN SLACK.
3. Father shall have par1ial physical custody of the minor child at those times as may he
mutually agreed upon hetween himself and the matemal grandparents.
4. Mother shall have panial physical custody of the minor child at those times as may be
mutually agreed upon between herself and the matemal grandparents
5. All panics to this agreement. including the Maternal Grandparents. shall have the authority
to make medical decision for the child amI to direct medical personnel to render appropriate medical
al1ention. All panies agree to inform the other panies of any medical emergency as soon as practicable.
6. Matemal grillulparents shall not unreasonahly withhold rights of visitation or temporary
physical custody from either paren!.
