follows:

have resided at the following locations during the five years next preceding the
filing of this petition:

1. The intended purpose for this change is to have my name reflect my
upcoming union with Kendra Alayne Fahnestock on October 12. 1997. Both Ms.
Fahnestock and I f,,'C1 a strong connection to our surnames and the hislory behind
them.
3. With this change of name. I am neither trying to avoid financial
responsibilities. nor am I attempting to defraud any enlity or individual. I will,
upon a decree entered by' this Court. contact all creditors 10 whom I am in debt and
instruct lhem that my name has been legally changed.
