The Petitioner, Laura Kate Anderson has resided at the following addresses for and during a period of five (5) years prior to the date of the filing of this Petition: June, 2006 to Present: 750 Dogwood Terrace, Boiling Springs, PA 17007 March, 2003 to June, 2006: 2420 Trillium Lane, Knoxville, TN 37920 August, 2002 to March, 2003: 4911 Holston Drive, Knoxville, TN 37914 May, 2000 to August, 2002: 2250 Northridge Drive, Jefferson City, TN 37760 3. The proposed new name of the Petitioner is: Vidya Laura Kate Anderson 4. The Petitioner has used the name of Vidya for the past twenty years. Vidya is the name she chooses to use. 5. The Petitioner has never been convicted of a felony. 6. The Petitioner has never been convicted of any of the crimes itemized in 54 Pa.C.S. 702 c(2).
