IV I wish to be buried in my family plot, which is located in Mount Olivet cemetery, Queens, New York. I want a suitable headstone of granite, with names, birth dates, and death dates, not only for myself, but for the additional six relatives whose names appear below and who are buried in that plot. Additionally, I wish to purchase perpetual care for that plot.
