the CUmberland/Perry Mental Health/Mental Retardation Cffioe.
2) Respondent is Fred Hoover, l"esiding at 927 West Trindle
Road, Mechanicsburg, Pennsylvania. Respondent is an adult individual
being presently 47 years of age with a birth date of April 21, 1948.
Attaohed hereto, marked as Exhibit "A", and inoorporated herein by
reference is a Petition prepared by the Cumberland County MH/MR
Offioe. Attached hereto, marked as Exhibit "B", and incorporated
herein by reference is notarized statement from Dr. Ira Paokman.
3) Petitioner requests that the Court appoint Harold Hoover and
Ruth Hoover, as guardians for the limited purpose of oonsenting to the
plaoement of Fred Hoover into the 2176 Waiver Program and to execute
any documentation needed to facilitate the plaoement of Fred Hoover
into said program.
WHEREFORE, Petitioner, Robert L. O'Brien, respectfully requests
that the Court order that Respondent Fred Hoover be adjudged an
incapaoitated person in reference to his placement in the 2176 Waiver
