Petitioner, Francis Lyman Reneker, is an adult individual residing at 1021 A West Trindle Road, Mechanicsburg, Cumberland County, Pennsylvania. 2. Petitioner was born March 29, 1957 in Harrisburg, Dauphin County, Pennsylvania. 3. Petitioner has been a resident of Cumberland County, Pennsylvania for at le&st six (6) months previous to the filing of this Petitio,. 4. Petitioner desires to change his name from Francis Lyman Reneker, which became his name when he was adopted in 1970, to Francis McGavery Smith, which was his birth name. 5. Petitioner does not wish to change his name for any illegal purpose, to avoid the payment of taxes or other debts, or to avoid any obligation. 6. Petitioner's counsel, Michael J. Hanft, has searched the index in the Prothonotary's Office in the Court of Common Pleas of Cumberland County and there are no judgments against Francis (yman Reneker. WHEREFORE, Petitioner, Francis Lyman Reneker, respectfully requests this Honorable Court to issue an Order changing his name from Francis Lyman Reneker to Francis McGavery Smith.
