5. That Petitioner's married name, during her marriage, from August
25, 1990 to the time of her divorce on November 5, 1996 had been MIRROTTO,
after which she assumed the surname of BANASZYNSKI.
6. That the Petitioner desires to change her name from PATRICIA
ANN BANASZYNSKl to ABBEY JILLIAN BRADLEY for the following reasons:
(a) Petitioner's mother and close friends call her ABB, (short
for ABBEY), after her aunt's name, and have done so since she was a youth.
(b) Petitioner desires that her middle name be changed from
ANN to JILLIAN simply as a matter of personal choice.
(c) Petitioner desires to change her last name from BANASZYNSKI
to BRADLEY, which is the surname utilized by her nephew and two uncles.
(d) Petitioner contends that the name BANASZYNSKI is cumbersome
and difficult to pronounce while engaging in her occupation which entails
dealing with the public on the telephone and in her social life.
7. That there are no minor children who would be affected by the
above referred to name change.
8. That Petitioner is unremarried, having been divorced on November
S, 1996.
9. Petitioner vas discharged in a Chapter 7 Bankruptcy on April 23,
1987 under the name of PARTICIA A. ~~ASZYNSKl. A copy of the discharge i.
attached hereto and ..rked Exhibit A.
10. That Petttloner Is gainfully employed aa an Adalnistrative
Asalstant with the Pennaylvanta Department of State and that there are no
judgments or decree. of like character sgalnst the Pettttoner. A copy of
