TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of DARLENE ELAINE HAMILTON, PRO SE,
respectfully represents:
1. That I, DARLENE E, HAMILTON, am a resident of the
Commonwealth of Pennsylvania, residing at 55 Chelsea Lane,
Carlisle, Pennsylvania 17013
2. That I am Twenty-one (21) years of age.
3. That I am not now nor ever have been married.
4. That there are no judgments or decrees of like
character of record against me or my children.
WHEREFORE, I respectfully request the Court, pursuant
to the Act of December 16, 1982, P.L. 1309, No. 295, S 2, 54
Pa. Cons. Stat. S 702, to:
a. Fix a hearing on this Petition;
b. Direct that notice be given of this Petition and
hearing; and
c. Enter a decree changing my name from DARLENE ELAINE
HAMILTON to ELAINE ASSLYN HAMILTON.
