The plaintiff, Do-MOl Corp., trading as Embers Inn and Convention Center, is a Pennsylvania corporation, having a place of business at 1700 Harrisburg Pike, Carlisle, Cumberland County, Pennsylvania. 2. The defendant, Donna Y. Jones, doing business as First Step Montessori Academy, is an individual with an address of 51 l3altlmme Street, Carlisle, Cumberland County, Pennsylvania. 3. On May 30, 1996, the defendant had restaurant charges of $1,486.31, as shown on the copy of the statement attached, marked Exhibit "A". 4. The defendant has made the following payments: 10/22/96 3/18/97 4/2B/97 $ 100.00 100.00 500.00 700.00 $ This left a balance still owing of $786.31. 5. Despite repeated requests, the defendant has failed to make payments on this b<llance due. WHEREFORE, the plaintiff claims of the defendant the sum of $786.31, an amount requiring compulsory referral to arbitration under the local Rules of Court.
