section 3301(C) of the Pennsylvania Divorce Code.

was filed on September 10, 1997, and was served on Hadley Ross
Cushner, via United States, first class, certified mail, Restricted
delivery, return receipt requested on September 22, 1991.
3. Date of execution of the affidavit of consent: by

4. Date of waiver of notice of intention to request entry of
a divorce decree: by Plaintiff January 2, 1998; by Defendant

The Plaintiff, Mary Kathryn Cushner, by and through her
attorney, Clayton W. Davidson, Esquire, hereby files this complaint
against the Defendant, Hadley Ross Cushner, and avers the
following I
1. The Plaintiff, Mary Kathryn Cushner, is an adult
individual residing at 810 Charlotte Way, Apartment 101, Enola,
pennsylvania, 11025, Cumberland County.
2. The Defendant, Hadley Ross Cushner, is an adult
individual residing at 116 Captains Cove Drive, San Rafael,
California, 94903.
3. The Plaintiff and Defendant were married on November 23,
1996 in Enola, Pennsylvania.
4. The Plaintiff has been a bona fide resident of the
Commonwealth of Pennsylvania at least six (6) months immediately
prior to the filing of this Complaint.
5. There has been no prior action for divorce or annulment
of marri~ge between the parties in this or in any other
jurisdiction.
6. Neither party is a member of the armed 8ervice~.
