TO THE HONORABLE JUDGES OF SAID COURT: Petitioner, Michelle Ensminger on behalf of her minor child, Nicole McCarthy, by and through her attorneys, the Family Law Clinic, hereby petitions this Honorable Court to enter a decree changing her name from Nicole Lynn McCarthy to Nicole Lynn Ensminger and respectfully represents: 1. Petitioner, Nicole MCCarthy, was born on April 4, 1994 in camp Hill, Cumberland County, Pennsylvania. Petitioner's name at birth was Nicole Lynn McCarthy. A copy of her original birth certificate is IIttllched hereto as Petitioner's Exhibit "A" and incorporated herein by reference, 2. petitioner, Nicole McCarthy, currently resides at 319 3rd street, West Fairview, Cumberland County, Pa, 17025. 3. Nicole McCarthy received the following social security number under the name Nicole McCarthy: 202-74-9712. 4. Petitioner, Michelle Ensminger, on behalf of her minor child, Nicole McCarthy, seeks to change her name to Nicole Ensminger, for the following reasons: a) At the time of her birth Nicole was given the name of McCarthy. b) It was believed that Mr. Edward McCarthy, Jr. was Nicole's biological father. c) Edward MoCarthy, Jr. resides at 23 Hemlock Lane, Etters, PA 17319. d) A paternity test has shown that Edward McCarthy, Jr. is not Nicole's natural father. A copy of the paternity test is "ttached hereto as Petitioner's Exhibit "B" and incorporated herein by reference. e) Michelle Ensminger has had no contact with Edwa:::d McCarthy, Jr. and does not hold him out to be the father ot her child. Nicole has had no contact with Edward McCarthy, Jr. f) Edward McCarthy, Jr. does not consider Nicole to be his daughter and lias shown no interest in the child. g) Edward McCarthy, Jr. will consent to Nicole's name change. h) The name of Nicole's biological tather is Mr. Joseph Bernstein. i) Joseph Bernstein resides at 306 South York street, Mechanicsburq, PA 17055. j) Joseph Bernstein will consent to Nicole's name change and has no objection to changing Nicole's last name to Ensminqer. k) It is in the best interests ot Nicole McCarthy to have her mother's last name and not the name of McCarthy since Mr. McCarthy is not related to her. 5.
