The minor child, Brittany Weibley, WilS born on November 23/ 1986. Her Social Security Number is 197 - 66.0723. 3. The respondent is AAMOS Insurance. 4. The respondent, AAMOS Insurilnce is licensed to do business in the Commonwealth of Pennsylvilllia. 5. On August 15, 1995, the minor plaintiff, underwent il surgical procedure to have a tooth removed. During the procedure, the minor child suffered an accidental burn on her cheek which hilS left .1 permanent scar. 6. The procedure WilS performed by William Currie. 7. All medical bills hilve been p.lid and there are no outst,lnding bills or creditors that have not been p<lid. 8. AAMOS Insurance, the carrier for Williilm Currie, has agreed ti! pay One Hundred Thousand Doll<us ($100,000.00) to resolvl' ,lny and all claims the minor child may have against their insurl'd. 9.
