At the special instance and request of Oefendant,
Plaintitf provided advertising in its publication known as
the "V,llow Pages". After applying all proper credits,
thore l'l'emains a principal balance due of $3,655.41, as set ..~-.-_--.--.__._-___
forth in tho attached statement of account. As provided in
the attached advertisement contract, Plaintiff is als~
entitled to interest on the above amount from 06/13/96 to
07/16/97 to the agreed rate of 18% per annum totalling
$717.47 and its attorneys fees in the amount of $1,093.22,
for a total of $5,466.10, which, despite demand by
Plaintiff, remains due. Plaintiff maintains a place of
business in Philadelphia establi~hing a proper basis for
