At the special instance anij request of Defendant,
Plaintiff provided advertising in its publication known as Atty File #
the "Donnelley Directory". After applYing all proper 931678
credits, there remains a principal balance due of
$3,170.00, as set forth in the attached statement of
account. As provided in the attached advertisement
contract, Plaintiff is also entitled to interest on the
above amount from 93/04/18 to 95/07/31 at the agreed rate
of 18% per annum totalling $1,303.78 and its attorneys
fees in the amount of $526.22, for a total of $5,000.00,
which, despite demand by Plaintiff, remains due. Defendant
was required to forward all payments to Plaintiff in
Phil~elphia establishing a proper basis for venue in this
