WRIT OF ATTACIIMENT
The above employer shall attach and deduct from the wages of the above employee
a sun not to exceed ten (10%) of the net wages per pay period of said employee or a
sum not to place employee's net income below the poverty income guidelines as provided
annually by the Federal Office of Management and Budget, whichever is less. nNet
wagesn shall Jrean all wages paid, less only the following items:
1. Federal. state and local income taxes;
2. F.LC.A. payments and non-voluntary retirement payments;
3. Union dues; and,
4. lIealth insurance premiums
The amount of wages to be attached shall total $.l.O12.36 (plus costs)
The employer shall send the attached wages to the Prothonotary, Ct:mberland County
Courthouse. 1 Courthouse Square, Carlisle, PA 17013, payable to Plaintiff-Creditor:
John & r~nette Potteioer within fifteen (15) days from the close
of the last pay period in each month. The employer shall be entitled to deduct fran
the wages collected from the employee pursuant hereto the costs incurred from the
extra bookkeeping necessary t.o implement the terms within the Writ of Attac1TOent, not
exceeding $5.00 of the amount of the wages so deducted. If you. the employer, are
served with more than one Writ of Attachment for damages arising out of a residential
lease against the same employee. then the wage attachments shall be satisfied in the
order in which said Writs of Attachment were served_ Each prior wage attachment
shall be satisfied before any effect is given to a subsequent attachment.
You shall not take any adverse action against the employee solely because his
wages. salaries or commissions have been attached. Violations may result in (i) you
being adjudged in contempt and committed to jail or fined by the court and (ii) an
action against you by the employee for damages.
Willful failure to comply with this Writ of Attachnent ..nay result in (i) you
being adjudged in contempt of court and committed to jail or fined by the court;

WRIT OF ATTACHMENT
The above employer shall attach and deduct fran the wages of the above employee
a sun not to exceed ten (10%) of the net wages per pay pericx:I of said employee or a
sun not to place employee I s net incane below the poverty incane guidelines as provided
annually by the Federal Office of Management and Budget, whichever is less. nNet
wagesn shall mean all wages paid, less only the following items:
1. Federal, state and local incane taxes;
2. F.I.C.A, payments and non-voluntary retirement payments;
3. Union dues; and,
4 . Health insurance premiums
The amount of wages to be attached shall total $_1012.36 (plus costs)
The employer shall send the attached wages to the Prothonotary, Cumberland County
Courthouse, 1 Courthouse Square, Carlisle. PA 17013, payable to Plaintiff-Creditor:
John ~ r~npr~p Pn~~pigpr within fifteen (15) days from the close
of the last pay period in each rronth. The employer shall be entitled to deduct fran
the wages collected fran the employee pursuant hereto the costs incurred fran the
extra bookkeeping necessary to implement the terms within the Writ of AttacITnent, not
exceeding $5.00 of the amount of the wages so deducted. If you, the P.ll1ployer, aI'e
served with rrore than one Writ of Attachnent for damages adsing out of a I"esidentia1
lease against the same employee, then the wage attachnents shall be sntisfied in the
order in which said Writs of Attachnent ~re served. Each prior wage attachnent
shall be satisfied before any effect is given to a subsequent attachnent.
You shall not take any adverse action against the employee solely because his
wages, salaries or ccmnissions have been attached. Violations may result in (i) you
being adjudged in contempt and ccmnitted to jail or fined by the court and (il) an
action against you by the employee for damages.
Willful failure to comply with this Writ of Attachnent may result in (i) you
being adjudged in contempt of court and ccmnitted to jail or fined by the court;
(H) ycu being held liable for any amount not withheld, or withheld but not forwarded
to the Prothonotary's Office; and, (Hi) attachnent of your funds or property.
This Writ of Attachnent has been entered pursuant to 42 PA. C.S.A. S8127. as
amended by House Bill 908, Act 5 of 1996, effective February 15, 1996. A copy of this
Writ of Mtachnent has been sent by U.S. Mail, postage prepaid, to the .~nployee's

WRIT OF ATTACHMENT
The above employer shall attach and deduct fran the wages of the above employee
a sun not to exceed ten (10%) of the net wages per pay pericxl of said employee or a
sun not to place employee's net incane below the poverty incane guidelines as provided
annually by the Federal Office of Management and Budget, whichever is less. "Net
wagesn shall mean all wages paid, less only the following items:
1. Federal. state and local incane taxes;
2. F.I.C.A. payments and non-voluntary retirement payments;
3. Union dues; and,
4. Health insurance premiums
The amount of wages to be attached shall total $ 1012.36 _ (plus costs)
The employer shall send the attached wages to the Prothonotary, Ct:mberland County
Courthouse, 1 Courthouse Square. Carlisle, PA 17013. payable to Plaintitf-Creditor:

of the last pay per.icxl in each rronth. The employer shall be entitlccl to dccluct frail
the wages collectccl fran the employee pursuant hereto the costs incurred fr::m the
extra bookkeepin(J necessary to implement the terms within the Writ of AttaclTllent, not
exceeding $5.00 of the amount of the wages so deductccl. If you. the employer, at'e
served with more than one Writ of Attachment for damages arising out of a residential
lease against the same employee. then the wage attachments shall be satisfied in th"
order in which said Writs of Attachment were servccl. Each prior wage attachment
shall be satisfied before any effect is given to a subsequent attachment.
You shall not take any adverse action against the employee solely because his
wages, salaries or carmi.ssions have been attached. Violations may result in (i) you
being adjlrlged 5.n contempt and cmmitted to jail or fined by the court and (El an
action against you by the employee for damages.
Willful failure to comply with this Writ of Attachment may result UI (i) you
being adjlrlged in contempt of court and ccmnitted to jail or fined by the court;

WRIT OF ATTACHMENT
The above employer shall attach and deduct fran the wages of the above employee
a sun not to exceed ten (10%) of the net wages per pay period of said employee or a
sun not to place employee's net incane below the p:>verty incane guidelines as provided
annually by the Federal Office of Management and Budget, whichever is less. nNet
wagesn shall mean all wages paid, less only the following items:
