PETITION FOR COMPROMISE SETTLEMENT OF MINOR'S CLAIM
NOW COMES CARLOn A LAMPKIN, by and through her legal guardian and grandmother
CARLOn A WILSON, and by counsel R. JACK CLAPP & ASSOCIATES, to petition this court
for a compromise settlement of her claim pursuant to Pennsylvania Rule of Civil Procedure 2039
and for the reasons more fully set forth in the following:
I, Carlotta Lampkin was injured while a passenger in an automobile that was involved
in a collision,
2. Carlotta Lampkin was thirteen years of age at the time of the accident and is now
sixteen years old and a single mother.
3. Carlotta Lampkin alleged that, due to the accident, she was entitled to personal injury
benefits through her grandfather and co-guardian's policy of insurance,
3. This policy of insurance was issued to him by State Farm Insurance Company.
4. Carlotta Lampkin fortunately suffered no pennanent disability.
5. She did, however, suffer damages including minor scarring, some pain, and medical
expenses which totaled approximately $932.00 (NINE-HUNDRED THIRTY-TWO DOLLARS).
6. State Farm, Carlotta Lampkin, her guardian Carlotta Wilson and their attorney
Douglas 1. Desjardins have come to an agreement whereby State Fann will pay $5,000,00 (FIVE-
THOUSAND DOLLARS) to settle her claim against it.
7. All of the parties herein mentioned agree that this amount is adequate and sufficient
compensation for her claim,
8, Carlotta Lampkin and Carlotta Wilson further understood and agreed that reasonable
compensation for their counsel in this matter, R. Jack Clapp & Associates, would be 1/3 (ONE

AFFIDAVIT OF CARLOTTA WILSON IN SUPPORT OF PETITION FOR
COMPROMISE SETTLEMENT OF MINOR'S CLAIM
After being duly sworn, Carlotta Wilson deposes and says:
1, My name is Carlotta Wilson and I live at 130 W, North Street, In Carlisle,
Pennsylvania,
2, I am the grandmother and legal guardian of Carlotta Lampkin,
3, I am Intimately familiar with the events and circumstances, Including the
nature of her injury, which led to my grandaughter's claim,
4. I have reviewed the attached petition and it Is a true and accurate
representation of my feeling In this matter.
5. I believe the settlement to be adequate compensation considering Carlotta's
limited Injury,
6. I understand the reduction for attomey's fees and costs and agree that they
are reasonable,
7. I am satisfied with the net settlement amount and believe that. Given her
situation as a new mother, the net funds are most appropriately paid directly
to Carlotta Lampkin,
FURTHER AFFIANT SAYETH NOT.
