PETITION FOR CHANGE OF NAME
AND NOW, comes Chasity Michele Lear, through her attorney
James K. Jones, Esquire, and brings this petition for Change of
Name, a statement of which follows:
1. petitioner Chasity Michele Lear is an adult resident of
Cumberland County, Pennsylvania residing at 154 E Street, Borough
of Carlisle, Pennsylvania.
2. Petitioner has resided at said residence for at least
the last five years.
3. Petitioner desires to change her legal name to Chasity
Michele Cornman as her natural mother has remarried and taken the
surname of "Cornrnan" and petitioner desires to have the same last
name as her mother and step-father.
WHEREFORE, petitioner requests this Court to schedule a
hearing on this matter and thereafter enter an Order of Court
changing Petitioner's name to Chasity Michele Cornman.
