The Petitioner, Monica Hayes, through her undersigned counsel, requests that this Court order a
Change of Name pursuant to 54 P.S. ~ 701 et. seq., and in support thereof represent as follows:
1. Pelltioner, Monica Lynn Hayes, is a resident of Enola, Pennsylvania, and has resided at the
following locations during the five (5) years next preceding the filing of this Pellllon: 48 Grant Street, Enola,
Cumberland County, Pennsylvania, from September 3, 1992 to present.
2. Petitioner, Monica Lynn Hayes, is the natural mother of the minor child, Kelsey Morgan Wicks
(O.O.B. August 27, 1994).
3. The minor child, Kelsey Morgan Wicks, has, since her date of birth, resided with both of the
undersigned Petitioner, Monica Lynn Hayes and her maternal grandmother, Linda Sue Hayes.
4. The natural father, Jeremy J, Wicks, has had absolutely no contact with the minor child,
Kelsey Morgan Wicks, since eight (8) weeks of age and has never Indicated any desire to assume any
