The petitioner, Susan EHzabeth Cioootta, has the desire and
intention to change her name and through her undersigned oounsel,
requests that this Court order a ohange of name for her, pursuant
to 54 Pa, Cons. stat, S701 et seq" and in support thereof
represents as follows:
1. Petitioner was born on Maroh 7, 1977 and is twenty-one
years of age,
2. Petitioner, Susan Elizabeth cioootta, is a resident of
Camp Hill, Pennsylvania and has resided at the following
10oations during the one and a half years preceding the filing of
this petition:
133 Fineview Road
Camp Hill, PA
17011
which has been her permanent residence sinoe 1997 and where she
lives with her grandparents;
20 Britton Road
Apartment E
Shippensburg, PA
17257
which is her residenoe while attending Shippensburg University;

where petitioner lived with her mother until she gradubted from
high school in 1995,
3, There are no judgments or deorees of like oharaoter of
reoord against the petitioner.
4, Petitioner requests that her name be ohanged from Susan
Elizabeth ciccotta to Susan Elizabeth Kaminski,
5. The reasons for the requested change of name are the

The petitioner, Susan Elizaheth ciccotta, has the desire and
intention to change her name and through her undersigned counsel,
requests that this Court order a change of name for her, pursuant
to 54 Pa, Cons. stat, ~701 et seq., and in support thereof
represents as follows:
1. Petitioner was born 011 March 7, 1977 and is twenty-one
years of age.
2. Petitioner, Susan Elizabeth Ciccotta, is a resident of
Camp Hill, Pennsylvania and has resided at the following
locations during the one and a half years preceding the filing of
this petition:

Camp Hill, PA
17011
which has been her permanent residencE! since 1997 and where she
lives with her grandparents;

which is her residence while attending Shippensburg University I

where petitioner lived with her mother until she graduated from
high school in 1995,
3. There are no judgments or decrees of like character of
record against the petitioner.
4. Petitioner requests that her name be changed from Susan
Elizabeth ciccotta to Susan Elizabeth Kaminski.
5. The reasons for the requested change of name are the
following:
(a) To conform petitioner's name with the name of her
maternal grandparents, Frank and Dorothy Kaminski, with whom
petitioner presently resides and has continuously resided since
approximately since September, 1997;
