date of birth and your Petitioners agree that they shall share
both legal and physical custody in the future.
5. The present address of the child for approximately 50%
of the time with her mother, your Petitioner, Tracy K. Hummel at
219 Clay Street, West Fair-view, Cumberland County, Pennsylvania
and approximately 50% of the time with her father, your
Petitioner, Stephen D. Hornbaker at 60 Sharon Road, Enola,
Cumberland County, Pennsylvania.
6. The child has resided at all times since her birth with
the following persons at the following addresses:
(a) From her date of birth until approximately
November 1995 at 60 Sharon Road, Enola, Cumberland County,
Pennsylvania, with your Petitioners and your Petitioner, Stephen
D. Hornbaker's, parents.
(b) From November 1995 until the summer of 1996
approximately 50% of the time with her mother in an apartment in
West Fairview and the other 50% of the time with her father and
his parents at 60 Sharon Road, Enola, Cumberland County,
pennsyl vania.
(0) From the summer of 1996 until the present
approximately 50% of the time with your Petitioner, Tracy K.
Hummel at 219 Clay Street, West Fairview, Cumberland County,
Pennsylvania and approximately 50% of the time with your
Petitioner, Stephen D. Hornbaker and his parents at 60 Sharon
Road, Enola, Cumberland County, Pennsylvania.
7. Your Petitioners have not participated as parties,
witnesses, or otherwise in any other litigation concerning the
custody of this child in any other state.
8. Your Pfltitioners have no information of any custody
proceedings concerning the said child in any court of
Pennsylvania or. in any other state.
9. Your Petitioners do not know of any person not a party
to this proceeding who has physical custody of the said child or
who claims to have oustody or visitation rights which respeot to
her.
10. Your Petitioners agree that they shall have joint legal
custody and shared physical custody in an approximate 50/50 split
as the parties may agree.
