1. Plaintiff Is John Whary addrell8 is 6221 Elmer A venue, Dauphin County, Harrishurg,
PA,17112.
2. Defendant Is BethZllda Rivera, 16011ST Floor Derry Street, Dauphin County,
Harrisburg, PA 17104.
3. Plaintiff Is an ownerofa 1985 Chryslerwhlch was damaged In Lemoyne, PA.
4. On September 3, 1997, Plaintiff's Chrysler was damagl.-d by Detendant's motor vehicle
serial number lHOAD7439BA040733.
5. Plaintiff's auto WI8 driven by Scott Whary, and the passenger was Chris White.
6. Plaintiff's auto WI8 damaged by fault of Defendant's auto.
7. Defendant's friend was ptlssenger In Defendant's auto at time of collision.
8. DefendAnt's friend offered the Insurance of Defendant's Premier Insurance and
admlUed fault.
9. Defendant's auto driver also admitted fault and offered only his operator's license.
10. Plaintiff has Incurred filing C05tB In the amount of $45.25.
11. Plaintiffs damages to auto were $615.86.
12. Plaintiff demands judgment against Defendant for costs and damages In the amount
of $661. 11 plus Interest on principal debt.
