AND NOW, the Petitioner, by her Attomey, Albert J. lIajjar,
petitions this Court, under authority of the Act of. 1982, P.L.
1309, No. 295, ~2, 54 Pa.C.S.A. ~/Ol at seq., for a change of
name based on the following allegations:
1. The Petitioner is Huth Ann Caparella, an adult, unremarried
widow residing at 592 Brighton Place, Mechunicsburg, Cumberland
County, Pennsylvania and has resided there during the five years
next preceding the filing of this Petition.
2. 'l'he Peti t.ioner was named at birth "Ruth Ann SalsberY." See
copy of official Birth Certificate attached hereto and marked
Exhibit "A."
3. On September 18, 1962. Petitioner was lawfully married to
one Ernest A. Caparella, whereupon she assulned her ne~ married

name, "caparella." See copy of Marriage certificate ~ttached and
marked Exhibit "D."

4. On August 24, 1982, said Ernest A. Caparella died in
Cumberland County, Pennsylvania, while being married to
Petitioner.
5. Petitioner has no children by her deceased husband Ernest A.
Caparella.
6. Petitioner now desires to assume her family name of
"Salsbery," to be known as "Ruth Ann Salsbery."
